REFERRALS
YES! We are accepting referrals.
By submitting a referral, it helps our programming team determine if and where an individual would be a good clinical fit in Sprout’s service array. We accept referrals and/or service authorizations from the Multi-Agency Alliance for Children, The Department of Children and Family Services and Private Pay families.
The information our team needs from the referring stakeholder includes:
- Behavioral Background & Concerns
- Diagnostic impressions
- School/Education Records
- Current symptoms
- History of treatment and medications
- Psychological assessments
We need some basic information from you to initiate services. Please complete a Referral Form to begin.

If there are any questions before booking an appointment, you can email basic information and contact information at referrals@sproutbehavioralhealth.org
We will contact all referred individuals within 48 hours of information receipt on Monday through Thursday to schedule an appointment. Submissions made on Friday will be processed on the following Monday.
